The incident began when a maroon car brought an area of the city centre to a standstill as the driver performed 'donut' manoeuvres while leaning out the window and shouting at onlookers.
As hundreds of people watched on, including police officers, the car then sped off down a busy road and mounted the pavement.
Dozens of victims were sent flying into the air.
Several witnesses reported seeing a mother with a baby in a pram being hit at speed.
According to Sky News Australia, the infant died at the scene.
Gunshots were heard before the maroon car came to a stop around two blocks away from where it had first stopped traffic.
The driver was seen being dragged from the wreck and given medical attention.
His condition is not known but Victoria Police said they arrested a man at the scene.
